はじめに

　われわれは前立腺にまで癌が浸潤していたにもかかわらず，膀胱癌を疑わしめる臨床検査成績がえられず病理組織学的に初めて診断できた症例を経験した。同例は結核性萎縮膀胱を基礎疾患としてもち，膀胱鏡検査ができなかつたことも診断を困難にした原因であるが，他にも種々の因子が関与していたのでその経過を報告する。

A 56 year old farmer, with the past history of orchiectomy for tuberculous epididymitis 24 years ago, left nephrectomy for renal tuberculosis 19 years ago and frequent urination 7 years ago, visited our clinic with the chief complaint of fever, hematuria and pain by micturition, had been treated for2 months without any improvement.

Urinalysis revealed hematopyuria with bacteria and leucocytosis was present. Acute distress including tenderness at bladder region was noticed and contracted bladder, vesicoureteral reflux and right hydro-nephrosis was found by X-ray examination.
