動脈撮影による腎障害について—症例報告および文献的考察

RENAL DAMAGE FOLLOWING AN AORTOGRAPHY:A CASE REPORT WITH A REVIEW OF THE LITERATURE

はじめに

　腎動脈撮影法は周知のごとく，現在ではDosSantos^1）以来の経腰法およびSeldinger^2,3）改良による股動脈法が広く用いられている。著者らはSeldinger法を好んで用いているが，最近，腎盂腫瘍症例にこの方法を用いて腎動脈撮影を行なつたところ，明らかに造影剤注入による一側性腎障害の発生を経験した。この症例の障害腎の病理学的検索とその発生機序について検討し，動脈撮影による腎障害について文献的考察を試みたので報告する。

Unilateral renal damage following aortography was observed in a case of 48 year old female with right renal pelvic cancer.

Despite the blood pressure throughout the procedure was as low as 90/70mm of Mercury, aor-tography by Seldinger's method was performed infusing Sodium Iothalamate (20ml, of a 80 per cent solution) as contrast medium. Subsequently, Megulmin Iothalamate (3ml of a 60 per cent solution) was manually injected into the right renal artery to obtain the selective renal-angiogram.
