膀胱瘻長期留置患者に発生した膀胱扁平上皮癌

A CASE OF BLADDER CANCER IN A PATIENT WITH LONG TERM CYSTOSTOMY

　69歳，男性。交通事故により生じた尿道損傷に対して行った手術が不成功に終わり，膀胱瘻にて生活していた。膀胱瘻の留置カテーテルが頻回に詰まるようになったため精査したところ，膀胱腫瘍が認められ根治手術を施行した。病理検査の結果はSCC＞＞TCC,pT3b，lNFβ，pLO,pVO,pNOで，非腫瘍部の粘膜は扁平上皮化生していた。膀胱内長期留置カテーテルによる発癌の危険性はよく知られており，十分な注意が必要と思われる。

A 69-year-old man was admitted to our hospital, because his cystostomy-tube was frequently obstructed. He had been with cystostomy for 13 years after the operation for urethral injury was failed. Transcystostomical cystoscopy and biopsy revealed bladder cancer, there-fore radical cystectomy was performed. Pathological diagnosis of the tumor was squamous cell carcinoma mixed with transitional cell carcinoma. Normally appear-ed bladder mucosa showed squamous metaplasia.
