残存憩室に発生した膀胱憩室扁平上皮癌

A CASE OF SQUAMOUS CELL CARCINOMA ARISING IN A RESIDUAL BLADDER DIVERTICULUM AFTER TRANS URETHRAL FULGURATION

　症例は72歳，男性。前立腺肥大症と尿道狭窄による多発性の膀胱憩室があり，経尿道的前立腺切除と狭窄切開をした後も尿路感染が持続し，憩室に対して経尿道的電気凝固術を施行した。3年後，残存した1個の憩室に腫瘍の発生を認めた。扁平上皮癌ですでに筋層浸潤し，膀胱全摘術を施行したが再発し死亡した。膀胱憩室腫瘍は予後不良とされているが，自験例も諸家の報告と同様であり，腫瘍発生を予防するために積極的な憩室の処置が必要と考えられた。

A 72-year-old man had multiple bladder diverticula with chronic persistent urinary tract infection due to benign prostatic hypertropy and urethral stricture. Although TUR -P and TUR-ST was done, urinary tract infection persist-ed. We performed transurethral fulguration against multi-ple bladder diverticula. However, three years later, a tumor arising in one residual diverticulum was detected. Squamous cell carcinoma with muscular infiltration was disclosed, therefore total cystectomy was done, but the patient died due to the recurrence.
