緒言

　精索は種々の組織で構成されており，そのいずれよりも腫瘍が発生しうる。しかしその報告例は決して多いとはいえず，岸本ら^1）によれば欧米における報告は良性腫瘍205例，悪性腫瘍123例，また本邦例は高村ら^2）が良性腫瘍44例，悪性腫瘍21例と集計報告している。その後の報告例および自験例を加えても，われわれは本邦の精索悪性腫瘍例を34例集めえたにすぎなかつた。

　精索腫瘍診断の第1歩は本疾患を疑うところにあるといわれてはいるが，病態分析が十分になされているとはいえないのが現状である。

A 75 year-old farmer, with a past history of left orchitis 30 years ago, was complaining pain from the nape to his right occiput for 2 months, and found an index-finger-tip sized indolent mass at the left inguinal region, accidentally. Patient visited our clinic with the chief complaint of this fast growing mass and dull pain at the lower abdomen.

The general condition was fair and inspite of some tenderness on the right side of the nape, no mass was palpable. No lymph node swelling was noticed.
