扁平上皮癌と移行上皮癌が隣接して発生した膀胱腫瘍

A CESE OF CONTIGUOUS SQUAMOUS AND TRANSITIONAL CELL CARCINOMA OF THE URINARY BLADDER

　61歳，男性．肉眼的血尿，排尿痛，頻尿を主訴として来院した．膀胱鏡検査で一部乳頭状，一部非乳頭状を呈する腫瘍を認めた．膀胱全摘術後の病理組織診断は，扁平上皮癌と移行上皮癌が各々純粋な組織を有する腫瘤として隣接して発生した膀胱癌であった．このような形の膀胱腫瘍は極めて稀であり，予後は非常に悪く，早期の根治的膀胱全摘術と術前放射線照射，術後化学療法を中心とする集学的治療が必要と考えられた．

A case is presented in which squamous cell carcinoma and transitional cell carcinoma coexisted as a contiguous pure mass. A 61-year-old male was evaluated for macroscopic hematuria with bladder irritability. Cy-stoscopy revealed two independent broad based tumors, one papillary and the other solid tumor, in the left lateral wall of the urinary bladder. Pathological diagnosis after total cystectomy was pure squamous cell carcinoma and transitional cell carcinoma which were coexisted contiguously. Both tumors were invasive into the muscle layer.
