われわれはβ-hCG産生膀胱癌の1例を経験した。患者は79歳，男性。肉眼的血尿のため当科を受診した。MRIでは，膀胱前壁に高度に浸潤した膀胱癌を認めた。経尿道的に切除した膀胱腫瘍の病理学的所見は移行上皮癌，G3であった。免疫組織染色では，hCG陽性の巨細胞を認めた。腫瘍の切除後ではあったが，血清β-hCGの上昇を認めた（0.9ng/ml）。腎機能障害があったため，追加治療としては化学療法ではなく放射線療法を選択した。放射線療法後は血清β-hCGは正常値まで下降したが，肝転移が出現した。診断後6か月の腹部CTでは，膀胱腫瘍に対する放射線療法の有効性が確認された。しかし，患者は診断後約7か月で癌死した。

Abstract We herein report the case of bladder carcioma producing β-hCG. A 79-year-old man visited our hospital presenting gross-hematuria. MRI showed an invasive bladder tumor at the anterior wall of the bladder. Pathological findings of the bladder tumor resected transurethrally were TCC,G3. Immmunohistochemical staining showed hCG positive giant cells. The serum β-hCG level obtained after transurethral tumor resection was elevated（0.9 ng/ml）. For the additional therapy,we selected radiotherapy not chemotherapy due to his renal dysfunction. After radiation,the serum β-hCG level returned to normal but liver metastases appeared. The abdominal CT at six months after the diagnosis showed that radiotherapy was effective for the bladder tumor. But,the patient died of the disease about seven months after the diagnosis.
