所謂皮膚混合腫瘍の1例

MIXED TUMOR OF THE SKIN

I．はじめに

　所謂皮膚混合腫瘍は，1892年Nasse^1）にょつて顔面鼻側の皮膚に発生した症例を内皮細胞性腫瘍として初めて報告されて以来，その特異的な，上皮性の腫瘍細胞巣と間葉性の粘液腫様ないし軟骨様組織が混在する，多形性をとる組織像から病理学者の興味をひき，それ以前に報告された，耳下腺を主とする唾液腺混合腫，後に発表された，涙腺に発生する涙腺混合腫瘍と共に，その発生病理が論ぜられて来た。現在では唾液腺混合腫瘍，皮膚混台腫瘍，涙腺混合腫瘍はそれぞれ，唾液腺，汗腺，涙腺の腺上皮から発生する異型腺腫であるとする，上皮説が一般に認められて来ているが，粘液腫様ないし軟骨様組織の発生機序については必ずしもいまだ一致していない。

　厳密にいえば，混合腫瘍とは2種以上の胚葉に由来する組織成分が，ともに腫瘍性性格を示す腫瘍であり，この考えから，増田・池田^2）らは，唾液腺混合腫瘍が多型性腺腫（pleomorphic ade—noma）とWillisらによつて改称されつつある様に，皮膚混合腫瘍を多形性汗腺腺腫　（pleomo—rphic hidradenoma）　とでも呼ぶことを提案している一方，これらの腫瘍に共通する特徴的組織像になじみの深い名称としての混合腺瘍の呼称をこれに"所謂"の名を冠する事によつて一応みとめている。

A 61-year-old housewife noted a miliary papule on the left nasolabial fold 5 months ago, which enlarged gradually to the size of a small-finger tip. The patient felt no subjective sensation. No swelling of the regional lymph nodes was proved.

The tumor was removed and a skin graft was performed. No recurrence has been noted in the course of the past one and a half year.

Histopathology of the tumor showed the typical picture of mixed tumor with a fibrous capsule in the upper and lower layers of the dermis. The tumor was composed of epithelial cells of padenoid type accompanied with myxomatous, cartilaginous and fibromatous tissues.

The Patient had the swelling of the thyroid and the basal metabolic rate was + 59%. The results of other laboratory tests and physical examinations were noncontributory.
