悪性変化を来した所謂耳下腺混合腫瘍の一例

　I．はしがき

　所謂唾液腺混合腫瘍が悪性化する場合のあることは，多くの人々の報告している事実だが，MacFarlandは総べて唾液腺混合腫瘍は潜在的には悪性のものと考えられると述べ，またH．RedonもDu pleissの意見に賛成し，混合腫瘍が悪性腫瘍であつて，その悪性化の出現はあらゆる時期に，如何なる前駆症状もなく突発し得るものであると警告している。しかし従来ややもすれば，この腫瘍を良性なものとして取扱う傾向にあり，その手術も単にenucleationに止める事が多かつたように思われる。その結果この疾患の予後を悲観的なものにしていた。

　私は最近，長年放置していた所謂耳下腺混合腫瘍が悪性化を来たし，癌組織に置き院換えられ，リンパ腺転移を来たした例を経験した。決して珍らしい症例ではないが，所謂混合腫瘍の治療には，北村教授等の努力によつて耳下腺外科が著しく進歩している現在，早期の保在的耳下腺亜全剔手術が最良の方法と信じられ，この一文を草した次第である。

Kaneko reports a case of malignant adenoma of the parotid gland that invaded the sur-rounding structures the malor region and the mandibular fossa but without involvement of the facial nerve. A man, aged 68, was affec-ted with a mixed tumor of the right parotid for a considerable number of years without any attention to the growth. The author em-phasizes the necessity of thorough and corn-plete removal of the parotid gland early in the course when such structures are found tobe affected with tumorous growths.
