　はじめに

　喉頭に発生する悪性腫瘍の大半は，扁平上皮癌であり，本邦の最近の報告によると，その98％を占めているとされる^1）。しかし，扁平上皮癌の亜型として分類されているsarcomatoid carcinomaが喉頭に発生する頻度は低く，喉頭悪性腫瘍のうち1％に満たない^2）。本邦における本疾患の報告例では，一般に喉頭の扁平上皮癌に準じた治療が行われており^3,4），とくに化学療法における薬剤選択などの工夫に乏しい。

　CYVADIC療法は，1975年にSouthwest On-cology GroupのGottliebら^5）により考案された，cyclophosphamide （CPM），vincristine （VCR），adriamycin （ADM），dacarbazine imidazole carboximide （DTIC）の4剤を用いる併用化学療法であり，悪性軟部腫瘍に対するレジメとして広く用いられている。われわれは喉頭のsar-comatoid carcinomaの2症例を経験し，化学療法としてCYVADIC療法を施行したので，その効果につき若干の文献的考察を加えて報告する。

The first case was a 66-year-old man presentinga tumor of the epiglottis with bilateral neck metas-tases. The second case was a 62-year-old man whohad a tumor of the right vocal cord with neck pain.For the biological characteristics of the sarcoma, CYVADIC chemotherapy, known as availablechemotherapy for sarcomas, was performed to thesecases. Inspite of remarkable response to CYVADIC, the first patient died of lung metastases, but thesecond one has been free from tumor for 19 months.
