　はじめに

　副咽喉頭間隙は，大血管や神経が走行し解剖学的に複雑な部位であり，多彩な病変の局在となる。われわれは，副咽頭間隙に発生した巨大腫瘤で術前に診断が困難であり，術後の病理組織学的所見で動脈瘤と確定診断された症例を経験したので，若干の考察を加えて報告する。

A 76-year-old male presented with a 30-years' history of a mass in the right tonsillar area. CTand MRI showed a large encapsulated mass occupied in the right parapharyngeal space ex-tending to the skull base. Angiography demon-strated a nonvascular mass displacing both the internal and external carotid arteries laterally. The patient underwent surgery under general anesthesia. After tracheostomy, the mass was extirpated through a lateral neck incision. Histo-pathological examination of the mass with HE, EVG and PTAH stains demonstrated an aneurysm.
