はじめに

　唾液腺の腺管内あるいは腺体内に形成された結石を唾石という。唾石が存在しても無症状に経過することもあるが，唾液腺や周囲の組織が炎症を起こすことがあり，これを唾石症と呼ぶ。唾石症は通常顎下腺に好発し，次いで耳下腺に認められるが舌下腺にできることは稀である^1）。また，唾石とは別に舌下腺に腫瘍ができる頻度も同様に稀であるが，舌下腺腫瘍は悪性である可能性が80〜90％と非常に高い^2）。本症例は口腔底の違和感で受診し，画像診断で舌下腺内に腫瘤を認めた1例である。舌下腺腫瘍の可能性を考えて舌下腺摘出術を行ったところ，術後の病理学的検査で舌下腺唾石症の診断に至った。文献的考察を加えて報告する。

　Sialolithiasis is one of the most common causes of salivary gland dysfunction. The submandibular gland is the most common site by far, and it rarely occurs in the sublingual gland. Herein, we report a case of a 26-year-old woman who presented to the hospital with discomfort in the floor of the oral cavity. Considering the possibility of a sublingual gland tumor, a sublingual adenectomy was performed, but a postoperative pathological examination revealed that the patient had sublingual sialolithiasis. Thin slice CT was considered important for the diagnosis of sialolithiasis.
