乾癬様皮疹の発症によりニボルマブ投与休止後に完全奏効状態を維持した再発舌癌症例

Successful treatment of tongue cancer recurrence with nivolumab, even after early discontinuation for psoriasiform eruption

はじめに

　免疫チェックポイント阻害薬は悪性黒色腫，肺癌，腎癌，尿路上皮癌などで有効性が証明されており，2017年3月より抗PD-1抗体であるニボルマブが「再発または遠隔転移を有する頭頸部癌」に対して保険承認された。免疫チェックポイント阻害薬は，時に免疫関連有害事象（immune related adverse event：irAE）が生じ，厳重な管理を要する場合もある。

　今回筆者らは，再発舌癌症例に対しニボルマブを4コース投与後，四肢・体幹に広がる皮膚症状が生じたため投与中止を余儀なくされたが，その後も再発病変が縮小しつづけ1年以上も完全奏効の状態を維持している症例を経験したので報告する。

　A 52-year-old man underwent two surgical treatments for tongue cancer. Later recurrence was treated with nivolumab；however, the drug was discontinued due to a prosiasiform eruption. After discontinuing the therapy, the recurrent lesions continued to shrink and a complete response was maintained even after one year.
