鼻性視神経症の治療経験と視力予後に関する考察

Treatment experience and visual prognosis in rhinogenic optic neuropathy

はじめに

　鼻性視神経症は，鼻副鼻腔疾患により視神経障害をきたす疾患群であり，失明に至る可能性のある重篤な病態である。その発症機序としては，副鼻腔囊胞などによる視神経への機械的圧迫，あるいは浸潤型副鼻腔真菌症などに伴う炎症の波及が挙げられる。早期に外科的治療を行うことで視力予後の改善が得られることが報告されており，手術の介入時期や術前視力が重要な因子とされている。

　鼻性視神経症に対して当科で緊急手術を施行した5例を後ろ向きに調査し，視力予後に影響を与える要因について若干の文献的考察を加えて報告する。

　Rhinogenic optic neuropathy is a severe condition caused by compression or inflammation of the optic nerve secondary to paranasal sinus disease. We retrospectively reviewed five patients who underwent emergency endoscopic sinus surgery at our institution. Four cases were due to paranasal sinus cysts and one to invasive fungal sinusitis. All cystic cases showed postoperative visual improvement, even in one with a 197-day delay before surgery. In contrast, the fungal case with no light perception did not recover. These findings suggest that surgical intervention may be beneficial for cystic lesions even after a prolonged interval from onset.
