はじめに

　喉頭蓋囊胞の感染から急性喉頭蓋炎や喉頭蓋膿瘍を発症する例は時に遭遇することがあるが，ガス産生性の喉頭蓋囊胞感染により発症する喉頭蓋膿瘍例は稀である。今回われわれは，ガス産生性の喉頭蓋囊胞感染から発症し，緊急気道確保を要した喉頭蓋膿瘍の1例を経験したので報告する。

　We report a case of epiglottic abscess caused by a gas-formed infection of the epiglottic cyst, requiring airway management. A 55-year-old man visited our hospital with chief complaints of sore throat and dysphagia. Using a flexible laryngoscope, a huge, yellow-colored, expansive, and swollen epiglottis accompanied by edematous changes in the bilateral aryepiglottic folds and arytenoids were found. Enhanced computed tomography revealed multiple air bubbles inside the swollen epiglottis. The airway was managed with an urgent endotracheal intubation in the emergency room, and tracheostomy was performed under general anesthesia. Following the management of acute infection, the residual epiglottic cyst was resected.
