ポリープ様声帯を疑った喉頭アミロイドーシスの1例

A case of laryngeal amyloidosis with suspected polypoid vocal cords

はじめに

　アミロイドーシスは，アミロイドと呼ばれる線維構造をもつ異常不溶性蛋白質が全身のさまざまな臓器に沈着し，機能障害を起こす原因不明の代謝性疾患である^1）。そのうち喉頭アミロイドーシスは限局性が主で，典型例では声門上部などに黄色調の粘膜下腫瘤を形成することが多く，ポリープ様声帯様病変を呈することは稀である。今回われわれは，その形状と色調から両側のポリープ様声帯を疑った喉頭アミロイドーシスの1例を経験したので，若干の文献的考察を加えて報告する。

　We report a case of laryngeal amyloidosis with polypoid vocal cord-like color and shape. A 64-year-old man visited our hospital with the chief complaint of hoarseness and throat discomfort. Physical examination revealed pale red polypoid lesions between the right ventricle and vocal cord and between the left vocal cord and the subglottis. These lesions were reduced by laryngomicrosurgery using the carbon dioxide laser under general anesthesia. Histopathologically, the diagnosis of amyloidosis was confirmed. Laryngeal amyloidosis should be considered in the differential diagnosis of laryngeal polypoid lesions with the aforementioned clinical presentation.
