妊娠中期に鼻腔より発生した化膿性肉芽腫の1例

A case of pyogenic granuloma occurring from the nasal cavity during mid-pregnancy

はじめに

　化膿性肉芽腫（pyogenic granuloma）は炎症性病変を伴う毛細血管の増生が主病変で，頭頸部や四肢に発生する血管腫の一種である。頭頸部の粘膜領域では口唇や口腔内に発生する場合が多く，鼻腔粘膜に生じる例は比較的稀とされている^1）。化膿性肉芽腫のうち妊娠中に発生する例は妊娠性肉芽腫などとも称され，女性ホルモンが増加する妊娠後期に発生することが多い。

　今回われわれは，妊娠中期に鼻腔内より発生した化膿性肉芽腫の1例を経験したので報告する。

　We report a case of pyogenic granuloma occurring in the nasal cavity during mid-pregnancy. A 43-year-old woman presented with recurrent left nasal bleeding that started in the 17th week of pregnancy. The dark-red tumor was located in the left anterior nostril. MRI revealed a mass in the left anterior nasal cavity with low intensity on T1 weighted image and high intensity on T2 weighted image.

　The pedunculated tumor was situated in front of the left inferior turbinate. It was resected with a bipolar electric coagulator under local anesthesia. Histological examination revealed a pyogenic granuloma. The postoperative course has been uneventful till date.
