上顎洞粘液腫の1例

Myxoma of the maxilla：Report of a case

I．はじめに

　粘液腫（myxoma）は，割面が特有の粘液質様の所見を呈する良性腫瘍であり，組織学的には，酸性ムコ多糖類とコラーゲンを主とする基質中に小型の紡錘形および星茫状の細胞の散在を特徴とする。基質中の線維成分の多いものは粘液線維腫（fibromyxoma）とも呼ばれる。腫瘍細胞の起源についての統一された見解はないが，胎生期の間葉系組織に由来する粘液芽細胞が主体との説^1,2）と，近年有力な線維腫あるいは線維性病変の粘液変性^3,4）との説に大別されている。

　粘液腫は身体各部に発生し，特に左心房に好発することが知られるが，顎骨に発生する腫瘍としては稀である。今回上顎洞に生じた1例を経験したので報告する。

　The myxoma of the jaw bones is an uncommon neoplasm,constituted about 3％ of all odontogenic tumors. A 45-year-old woman was referred to our hospital in September,2004 for a 6-year history of slowly progressing paresthesia of the right cheek. Computed tomography（CT）showed a soft tissue mass filling the right maxillary sinus. On exposure via Caldwell-Luc approach,a grayish white solid mass occupied the maxillary sinus. The lateral maxillary wall was distorted,but the tumor invasion into the perichondrium was not seen. The tumor tissues revealed to be typical benign myxoma.
