I．はじめに

　耳下腺腫瘍の診断・治療については，先人の努力により多くの重要かつ有用な報告がなされた^1）。その結果，診断や治療上での基本方針には各施設で大きな相違はないものと考えられる^2～4）。しかし，実際に発生する腫瘍の組織型の多彩さや顔面神経との相対的な位置関係によりさまざまな問題が発生しうる。今回われわれは過去の耳下腺腫瘍症例を検討し，診断や治療上の問題点を抽出し改善すべき点について考察した。

In order to find diagnostic and therapeutic problems,we performed a retrospective chart review of 200 cases of parotid gland tumor. There were 153 benign and 47malignant tumors. Fine needle aspiration cytology correctly predicted malignant tumor 10 out of 17 malignant cases（59％）and 67-Galium scan correctly predicted benign tumor only 45 out of 118 benign cases（52％）. Postoperative facial palsy was observed in 3.3％ of benign and 25％ of malignant cases. We have to make a careful diagnosis with comprehensive findings of various diagnostic modalities. Before surgery,careful informed consent about possible surgical complications should be obtained.
