プロトンポンプ阻害剤が著効した声帯ポリープの1例

A rare case of vocal cord polyp treated with proton pump inhibitor（PPI）

I．はじめに

　胃食道逆流症（gastroesophageal reflux disease：以下，GERD）のなかでも耳鼻咽喉科領域の症状を訴える症例は，咽喉頭酸逆流症（laryngopharyngeal reflux disease：以下，LPRD）と呼ばれる。Sataloffは嗄声をGERDの食道外病変の第一に挙げており^1），近年，本邦でもLPRDと音声障害の関連について報告されている^2,3）。しかし声帯ポリープは，循環障害により生じ手術的切除が治療の中心となることもあって，LPRDと声帯ポリープの因果関係を明らかにした報告はない。

　今回われわれは，術後約1か月で再発をきたした声帯ポリープがプロトンポンプ阻害剤（以下，PPI）にて消失した症例を経験したので，文献的考察を加えて報告する。

A 50-year-old male presented with vocal cord polyp located on the left vocal cord which appeared to be as a whole. The polyp was removed. Pathological diagnosis was laryngeal nodule. Three weeks after the operation,the polyp recurred. Laryngopharyngeal reflux disease was suspected after taking a detailed history. PPI test was done. In two weeks,the polyp disappeared. It was considered that PPI could be one of the treatments when the polyp is suspected to be associated with acid regurgitation or a patient refuses operation.
