当科で経験した副耳下腺腫瘍の3例

Three cases of accessary parotid gland tumors

はじめに

　副耳下腺はステノン管に沿って，主耳下腺から独立して耳下腺前方および咬筋外側に位置する異所性の唾液腺である。日本人（成人）の約30〜70％に存在する^1）と報告されており，耳下腺本体と同じ組織からなる。

　発生する腫瘍の病理組織型は耳下腺腫瘍と同様であるが，その発生率は全耳下腺腫瘍のうち1〜7％ときわめて少ない^2）。また，耳下腺腫瘍における悪性腫瘍の頻度が15〜30％程度であるのに対して，副耳下腺腫瘍では42〜52％と頻度は高い^3）。当院において副耳下腺腫瘍を3例経験したため，文献的考察を加えて報告する。

　Accessary parotid gland tumors are clinically rare；however, the rate of malignant tumors of the accessory parotid gland is approximately 42-52％, which is higher than that of parotid gland tumors. Therefore, a correct diagnosis and appropriate treatment should be selected. Three patients were diagnosed with accessory parotid gland tumors at our hospital between April 2017 and March 2022 and underwent resection using a standard Blair incision. In all cases, no facial nerve damage was observed after surgery. Two cases were diagnosed as pleomorphic adenomas, and the other as adenoid cystic carcinoma. No evidence of recurrence or metastasis was observed.
