耳下腺腫瘍鑑別診断基準についての検討

A Trial of Criteria Making for Ultrasonographic Diagnosis of Parotid Tumors

I．はじめに

　超音波診断装置の高性能化に伴い表在腫瘤の超音波検査の診断的意義も増し，件数も増加しつつある。現在表在腫瘤のうち乳腺については超音波診断基準が確立されている^1）が，耳下腺腫瘍の超音波診断基準は提唱されている^2,3）もののいまだ確立されたものではない。そこで今回私どもは耳下腺腫瘤を病理組織学的に分類して，その各群の超音波所見によるレーダーチャート図を作成し，それぞれの特徴についてまとめ，若干の検討を加えたので報告する。

A radar chart was prepared in an attempt to differentiate parotid tumors in patients who re-ceived ultrasonography during a 3-year period from 1984 to 1986.

The apparatus used was an electronic linear ultrasonograph equipped with a 5 MHz probe, using the water immersion method.

Classified patterns of a bottom area and intra-phymatoid non-echoic area were added to 7 items of diagnostic criteria for ultrasonography of the mammary glands and the findings were graded by scores.

The scores for malignant tumors based on thecriteria of ultrasonographic diagnoses were clas-sified by the following 3 items.

(1) unperceivable boundary echoes

(2) uneven internal echoes

(3) weakened posterior echoes
