頭頸部再建外科における遊離組織移植の導入—初心者に有用な血管吻合手技について

Technique of Free Tissue Transfer for Surgical Trainees

　はじめに

　頭頸部領域の再建には遊離組織移植が必須であるが，微小血管吻合の技術が必要でありその実施は必ずしも容易ではない^1）。一方，先人たちの努力により安全で確実な遊離移植組織が開発され積極的な施行が推奨されている^2）。われわれは，再建症例の増加に伴い，当科単独による遊離組織移植を導入した。導入当初は最も問題が多く，この時期を振り返ることは今後この手技を導入しようとする他施設の参考になるものと考えられる。われわれの血管吻合手技の概略を示し遊離組織移植導入時の症例を検討し問題点を探り考察を加えて報告する。

For free tissue reconstruction of head and neck with microvascular anastomosis, the triangular method of microvascular end to end anastomosis is employed to avoid through stitch of the opposite side of the vascular wall.

To estimate our technique, surgical cases were reviewed retrospectively. Nine out of 10 cases were successfully reconstructed. Success rate was compa-rable to the previous reports.

The triangular method of microvascular anas-tomosis is satisfactory because of high patency rate. The better surgical results can be obtained as the importance of the flap selection, design, and post-operative management are considered.
