同時期耳下腺外ワルチン腫瘍について

Synchronous Extra-parotid Warthin's Tumor

　はじめに

　ワルチン腫瘍（リンパ腺腫）は全耳下腺腫瘍の6〜10％に見られる発育緩慢でしばしば無症状に経過する良性腫瘍であるが，時に両側発生や同側の多中心性発生をみる特異な腫瘍である^1,2）。多中心性腫瘍は同時期発生のこともあるが，時期を異に発生することもある。したがって外科的切除後の同側発生腫瘍では，異時性多中心腫瘍であるのか再発性腫瘍であるのかは区別がむつかしくなるが，ワルチン腫瘍は耳下腺内のみでなく，耳下腺辺縁リンパ節や頸部リンパ節からの発生をみることもあり^3,4），この場合はその判断は容易である。今回私達は，同側耳下腺内の複数腫瘍と辺縁リンパ節様腫瘍が組織学的に同一のワルチン腫瘍であった，いわゆる同時期耳下腺外ワルチン腫瘍を経験したのでその症例を紹介し，文献的考察を加えて報告する。

A case of extra-parotid Warthin's tumor occurring synchronously in a peri-parotid lymph node was described. A 63-year-old man pre-sented with a three-month history of a painless, slowly growing mass below his left ear. There was no cervical lymphadenopathy on palpation or facial nerve involvement. At operation a discrete tumor was found in the lower pole of the parotid gland along with a adjacent lymph node. A superficial parotidectomy was carried out with complete excision of the tumor. Thehistological studies confirmed the diagnosis, and the lymph node was discovered to contain the same tumor. This was thought not be a me-tastatic phenomenon but the result of salivary gland inclusions of local lymph nodes during the embryological development of the parotid gland.
