ぶどう膜炎を伴った眼窩偽リンパ腫の1例

A case of orbital pseudolymphoma with uveitis

　45歳女性が左眼の下眼瞼腫脹，結膜充血，眼球運動に伴う違和感で受診した。矯正視力は右1.2，左0.6で，左眼に眼球突出，球結膜充血，外転と下転障害があった。中間透光体と眼底には異常がなかった。画像診断で左眼外眼筋の下方に浸潤性の腫瘍があった。眼窩腫瘍亜全摘出術を行い，病理組織学的には偽リンパ腫（reactive lymphoid hyperplasia）であった。角膜後面沈着物を伴う前部ぶどう膜炎が手術の8日後に左眼，1か月後に右眼に生じた。眼窩腫瘍は4回再発したが，プレドニゾロンの全身投与で腫瘍は縮小し，眼球突出は消失し，眼位と眼球運動は正常化した。約2年後に前部ぶどう膜炎が消失し，最終視力は左右眼とも1.0であった。悪性腫瘍の眼外転移の所見はなかった。二原的にぶどう膜炎が発症したと推定する所見はなく，眼窩偽リンパ腫が前部ぶどう膜に浸潤したものと考えられた。

A 45-year-old female presented with swollen lower eyelid, conjunctival hyperemia and difficulty in eye movement in her left eye. Her corrected visual acuity was 1.2 right and 0.6 left. The left eye showed proptosis and limitation of lateral and lower gaze. Diagnostic imaging showed an infiltrative tumor inferior to the lateral rectus muscle in the left orbit. Subtotal extirpation of the tumor led to the diagnosis of pseudolym-phoma, or reactive lymphoid hyperplasia. Anterior uveitis with keratic precipitates appeared in the left eye 8 days after surgery and in the right eye 3 weeks later. The orbital tumor recurred four times. Systemic predni-solone induced eventual regression of the tumor followed by disappearance of proptosis and normalization of eye position and eye movement. The anterior uveitis subsided 2 years later. Systemic examinations showed no extraocular invasion of tumor nor underlying cause of uveitis. It appeared that the uveitis was secondary to intraocular invasion of orbital pseudolymphoma.
