眼窩炎性偽腫瘍の7例

Seven cases of orbital inflammatory pseudotumor

　眼窩炎性偽腫瘍とは臨床上眼窩腫瘍の症状を呈するが，その経過，或は摘出腫瘍の組織学的所見は真の腫瘍と異り，炎症性機転により生じ，而も発生原因の不明のものを一括して総称した名称とされている。本症は1850年Flarer氏が始めて記載し，当時の報告者は本症の本態に関して少なからず，困惑させられたが，その後Birch-Hirsch-feld氏は始めて本症を定義し，病理学的にはあまり意味がないが臨床的内容を有する眼窩偽腫瘍なる名称を与えた。更に1910年には本症の50例を集めて総括記載し，分類を試みた事はよく知られている^1）。本症の原因本態は尚不明であるが，臨床経過，予後，治療に関しては明らかに真性腫瘍とは異るにも拘らず，両者の鑑別は困難の事が多く，臨床的にも本症の解明は意義深く興味のもたれる所である。私は当教室に於いて昭和28年より現在迄に観察された本症の7例について報告する。

Seven cases of orbital inflammatory pseudotumor are investigated and clinical course, biopsy and therapy are described.

Clinical symptoms and courses of pseudotumor are variable. Mostly patients are affected with advancing age. Proptosis (in all) is the outstanding sign, masses are palpable at the orbital outlet in 6. Disturbances of vision and ocular movement are noted in 5. There are no specific changes in the fundi, though macular edema, folding of the retina and pa-pilloedema are found in some cases. Blood count, serological test and radiography appear mostly normal.
