鈍的外傷による角膜内皮障害

Corneal endothelial damage after blunt ocular trauma

　片眼の鈍的外傷による眼球打撲52症例を対象とし，鈍的外傷が角膜内皮に及ぼす影響を検討した。受傷眼と健常他眼の角膜内皮細胞を，非接触型スペキュラーマイクロスコープを用いて測定した。角膜内皮障害の指標として角膜内皮細胞密度，変動係数，六角形細胞出現率を用いた。全症例での比較，あるいは受傷原因（交通事故，手拳，ボール）別や出現した臨床症状（前房内炎症，相対性求心性瞳孔反応欠損，網膜振盪症，眼窩骨折）別に比較した。全症例，受傷原因別，出現した臨床症状別のすべてで，受傷眼と健常他眼の間に3つのパラメーターで有意差はみられなかった。角膜内皮は比較的軽傷の鈍的外傷による瞬間的な角膜の変形に対して安定であることが示唆される。

We evaluated the possible damage to the corneal endothelium in 52 cases with unilateral blunt trauma. We used a noncontact specular microscope to study the corneal endothelial cells in the affected and the intact fellow eyes. Calculated parameters included cell density, coefficient of variation of cell area, and per-centage of hexagonal cells. We also assessed the causative lesions and clinical features. The former included traffic accident, fist or flying ball hitting the eye. The latter included signs of inflammation in the anterior chamber, relative afferent pupillary defect, commotio retinae, and orbital fracture. There was no significant dif-ference in the parameters between the pairs of eyes either in the whole series, causative lesions, or clinical features. The findings show that the corneal endothelium remains intact after slight blunt trauma with mo-mentary corneal transformation.
