エキシマレーザー屈折矯正角膜切除の角膜内皮細胞への影響

Corneal endothelial changes after excimer laser photorefractive keratectomy

　エキシマレーザー屈折矯正角膜切除photorefractive keratectomy （以下PRK）の角膜内皮細胞に対する影響をスペキュラーマイクロスコープを用いて検討した。PRKを施行した20例26眼の術前と術後1か月目に角膜内皮細胞のスペキュラー像をスペキュラーマイクロスコープを用いて撮影した。角膜内皮細胞の平均密度±標準偏差は術前3221±216／mm²，術後1か月3233±240／mm²，変動係数は術前0.24±0.09，術後1か月0.23±0.05であった。平均細胞密度，変動係数とも術前後で有意な差はなかった。スペキュラーマイクロスコープによる術後1か月の観察では，PRKによる角膜内皮細胞への明らかな傷害は認められなかった。

We evaluated the changes of corneal endothelium in 26 myopic eyes before and 1 month after photo-refractive keratectomy by an ArF excimer laser. The endothelial cell density averaged 3211±216/mm² before and 3233±240/mm² after surgery (mean±SD). The coefficients of variation of mean cell density was 0.24±0.09 befor and 0.23±0.05 after surgery. There were no significant differences between preoperative and postoperative values in endothelial cell density or coefficients of variation. These findings suggest that photorefractive kera-tectomy with excimer laser does not significantly affect the corneal endothelial cell density at 1 month after treatment.
