We quantitated the irregular corneal astigmatism following cataract surgery in 60 eyes of 60 patients. The eyes were randomized into three groups : 6.0 mm-, 4.1mm- and 3.5 mm-incision groups. Corneal topography was performed before and 1 month after surgery. The data were subjected to Fourier series harmonic analysis to calculate spherical component, regular astigmatism, decentration and higher-order irregularity. Eyes in 6 mm-incision group showed postoperative increase in regular astigmatism (p = 0.026, paired t-test) and higher-order irregular astigmatism (p=0.018) d. The other two groups showed no significant changes. Higher-order irregular astigmatism showed a significant postoperative intergroup difference (p < 0.05, ANOVA) . The difference was significantly higher in the 6 mm-incision group than in scleral 4.1 mm-incision group (p< 0.05, Bonferroni test) . Smaller incision cataract surgery thus induces lesser regular and irregular corneal astigmatism.
