We measured the refractive changes in the posterior corneal surface in 25 eyes of 15 patients who underwent excimer laser photorefractive keratectomy. A scanning-slit corneal topography (OrbscanTM, Orbtek) was used in the measurement. Posterior corneal refractive power averaged -6.56 diopters (D) preoperatively. It decreased to -7.02 D, -7.00 D and -7.05 D at postoperative 1 week, 1 month and 3 months respectively. The changes were significant (p < 0.001, paired t-test) . The amount of posterior corneal refractive changes amounted approximately to 10% of the total myopic correction. A signifincant correlation was present between the amount of refractive change and the residual corneal thickness (Pearson correlation coefficient r = 0.617, p < 0.001) . The findings show that photorefractive keratectomy induces significant changes in the posterior corneal refraction.
