A 33-year-old female presented with dizziness since one month before and diplopia since 10 days before. She showed nystagmus and right abducens palsy with diplopia. Magnetic resonance imaging (MRI) showed multiple high-signal lesions in the white matter around the lateral ventricles and in the right cerebellar nuclei. These findings led to the diagnosis of multiple sclerosis. Abducens palsy and diplopia re-solved 3 months after two courses of methylprednisolone pulse therapy. The MRI findings remained un-changed. This case illustrates that methylprednisolone pulse therapy may be effective for monocular ab-ducens palsy manifesting as an uncommon complication of multiple sclerosis.
