目的：過去に緑内障手術を1回以上受けた後に眼圧コントロール不良となった症例に対するシヌソトミー併用トラベクロトミーの手術効果を検討する。
対象と方法：手術後少なくとも6か月間経過を観察できた5例7眼である。症例は年齢15〜46歳（平均：28.2±11.8歳）の隅角発育異常緑内障で，全員男性であった。過去の緑内障手術は線維柱帯切除術1回が2眼，MMC併用線維柱帯切除術1回が1眼，線維柱帯切除術1回＋MMC併用線維柱帯切除術1回が1眼，線維柱帯切開術1回が2眼，線維柱帯切除術1回＋線維柱帯切開術1回が1眼であった。
結果：術前は全例内服治療と点眼治療を受けており，平均眼圧は32.9±5.2mmHgであった。最終観察時の術後眼圧は平均16.6±2.9mmHgであった。前房出血は全例14日以内に消失した。
結論：シヌソトミー併用トラベクロトミーは隅角発育異常緑内障に対する追加手術として有効である。
Purpose : To evaluate the outcome of combined surgery by trabeculotomy and sinusotomy in eyes with failed glaucoma surgery.
Cases : Seven eyes in 5 patients had goniodysgenetic glaucoma. The ages ranged from 15 to 46 years.
Previous surgeries included : trabeculectomy 2 eyes, trabeculectomy with mitomycin C (MMC) 1 eye, trabeculectomy followed by trabeculectomy with MMC 1 eye, trabeculotomy 2 eyes, trabeculectomy followed by trabeculotomy 1 eye. The intraocular pressure (I0P) averaged 32.9±5.2 5.2 mmHg with systemic and topical medications.
Results : After 6 months or longer following the combined surgery, the IOP averaged 16.6±2.9 2.9 mmHg under topical medications. Postoperative hyphema disappeared in all eyes within 14 days.
Conclusion : Combined trabeculotomy and sinusotomy was effective in eyes of goniodysgenetic glaucoma after failed glaucoma surgery.
