We evaluated the longterm effects of trabeculotomy with or without sinusotomy in 74 eyes of glaucoma. Trabeculotomy with sinusotomy was performed in 29 eyes of primary open angle glaucoma (POAG) and 6 of pseudoexfoliation glaucoma (PE) . Trabeculotomy alone was performed in 35 eyes of POAG and in 4 of PE. Preoperative intraocular pressure (IOP) averaged 23.4 ±5.6 mmHg in the former group and 23.1±3.4 mmHg in the latter. There were no significant differences. The IOP during 3 years after surgery was consistently and significantly lower in the former than in the latter group. When the successful postoperative IOP level was defined as 14 mmHg and when the Kaplan-Meier life table method was applied,the success rate was significantly better in the former than in the latter group. At the end of follow up, IOP level of 14 mmHg or lower was attained in 20 of 35 eyes (57%) in the former group and in 3 of 39 eyes (8%) in the latter. The findings show the superiority of trabeculotomy with sinusotomy for glaucoma over trabeculotomy alone.
