要約　シヌソトミー併用トラベクロトミー（SIN＋LOT），超音波乳化吸引術（PEA），眼内レンズ挿入術（IOL）（S群）102眼とLOT＋PEA＋IOL（L群）224眼の術後眼圧の比較検討を行った。L群の術翌日以外は全ての眼圧が術前に比し有意に低下しており，投薬点数も有意に低下していた。両群間では，術翌日から有意にS群が低下していた。術後18mmHg以下の生存率は術後42か月で，S群98％，L群64％，術後16mmHg以下の場合は，S群72％，L群21％であり，どちらも全ての期間でS群が有意に高かった。SIN＋LOT＋PEA＋IOLは視神経障害の進行した緑内障眼に有用な手術法と考えられた。

Abstract.　We reviewed the outcome of simultaneous cataract and glaucoma surgery in 326 eyes. The series comprised primary open-angle glaucoma 278 eyes and capsular glaucoma 48 eyes. Cataract surgery was performed by phacoemulsification-aspiration with intraocular lens implantation. Trabeculotomy was performed in 224 eyes and trabeculotomy with sinusotomy in 102 eyes. After surgery,the intraocular pressure（IOP）was significantly lower than the preoperative level in all the eyes（p＜0.001）except on day 1 in the trabeculotomy group. At 42 months of surgery,the IOP remained below 18 mmHg in 64％ in trabeculotomy group and in 98％ in sinusotomy group. The IOP remained below 16 mmHg in 21％ in the former and in 72％ in the latter. The difference between the two groups was significant until 30 months of surgery（p＜0.001）and at 36 months of surgery（p＜0.02）. The findings show that cataract surgery with trabeculotomy and sinusotomy is useful for eyes with cataract and glaucoma.
