続発性緑内障に対してシヌソトミー併用トラベクロトミーを行った3例

Trabeculotomy with sinusotomy for secondary glaucoma in three cases

目的：続発緑内障に対してシヌソトミー併用トラベクロトミーを行った3症例の報告。症例：眼サルコイドーシスの62歳男性，原因が不明なぶどう膜炎の74歳女性，フックス虹彩異色性毛様体炎の55歳男性それぞれの片眼にシヌソトミー併用トラベクロトミーを行った。第3例には白内障手術を同時に行った。術後2年以上の観察で，全症例で眼圧がコンロールされ，視野の進行がなかった。結論：シヌソトミー併用トラベクロトミーは術後の管理が容易であり，一定の効果を得ることができた。続発緑内障に対する手術の選択肢の1つになり得る。

Purpose：To report three cases of secondary glaucoma treated by trabeculotomy with sinusotomy. Cases：The presenet series comprised a 62-year-old male with ocular sarcoidosis,a 74-year-old female with uveitis of unknown etiology,and a 55-year-old male with Fuchs heterochromic iridocyclitis. All had glaucoma in one eye. They received trabeculotomy with sinusotomy. Case 3 received simultaneous cataract surgery. Control of intraocular pressure was obtained in all the 3 eyes for two years or longer. Visual field defect remained stationary in all the eyes. Conclusion：Trabeculotomy with sinusotomy was effective for secondary glaucoma. This method is recommended for selected cases.
