（18-P2-53）　1.0D以上の直乱視がある白内障11眼に，輪部減張切開術と耳側角膜一面小切開白内障手術を行った。術前の角膜乱視は2.0±0.8Dで，術後3か月での屈折乱視は0.5±0.5Dであった。角膜内皮細胞密度は9.2％減少し，全例で1.0以上の矯正視力が得られた。乱視軸の回転，過矯正，角膜穿孔などの合併症はなかった。本術式は，強い直乱視のある白内障に対して，安全で容易，かつ有効である。

We treated 11 eyes with cataract and with-the-rule astigmatism of 1.0 D or greater by limbal relaxing incision and phacoemulsification through a temporal clear corneal small incision. The preoperative corneal astigmatism averaged 2.0 ± 0.8 D. The refractive cylinder 3 months after surgery averaged 0.5 ± 0.5D. The corneal endothelial cell loss averaged 9.2%. All the eyes achieved corrected visual acuity of 20/20. There was no complication including axial rotation, overcor-rection or corneal perforation. The present combined technique was thus safe, easy and effective for cataract with pronounced with-the-rule astigmatism.
