We evaluated the longterm results of astigmatic keratotomy (AK) in 14 eyes with against-the-rule astigmatism of 3 diopters (D) or over after cata-ract surgery. The corneal astigmatism averaged 5.02 D before and 2.16 D 2 years after AK. The central corneal thickness returned to preoperativevalue 3 months after AK. The density of endothe-lial cells in central cornea did not change after AK.

The findings show that AK using transverse inci-sion was safe and efficient. There was a positive correlation between the amount of astigmatism before AK and the amount of astigmatism correct-ed by AK.
