　白内障手術後に3ジオプター（D）以上の倒乱視を有する13例14眼に対して，transver—se relaxing incision （T切開）による乱視矯正角膜切開術（AK）を施行し，術後2年間の経過を検討した。術後，全眼で角膜乱視の減少を認めた。平均乱視量は，術前5.02 D，術後2年目2.16 Dと，2.86 Dの減少を示した。角膜厚は術後3か月で術前値に復し，角膜中央部内皮細胞密度は術前後で変化を認めなかった。T切開によるAKは術後2年目において有効かつ安全であることが確認された。また，術前乱視量と乱視矯正重との間に正の相関があることがわかった。しかし乱視矯正量の予測が正確にはできない点、術後1年目以降も角膜乱視の変動を示す例がある点などの問題点もあった。

We evaluated the longterm results of astigmatic keratotomy (AK) in 14 eyes with against-the-rule astigmatism of 3 diopters (D) or over after cata-ract surgery. The corneal astigmatism averaged 5.02 D before and 2.16 D 2 years after AK. The central corneal thickness returned to preoperativevalue 3 months after AK. The density of endothe-lial cells in central cornea did not change after AK.

The findings show that AK using transverse inci-sion was safe and efficient. There was a positive correlation between the amount of astigmatism before AK and the amount of astigmatism correct-ed by AK.
