Abstract 文献概要

　光凝固後に解剖学的な治癒が得られた加齢性黄斑変性39眼について，術後視力に関係する要因を検討した。全眼で黄斑下に新生血管があった。術後観察期間は，6から78か月，平均13か月であった。術前視力が0.2以下の症例には中心窩を含めた新生血管全体を凝固した。この場合，新生血管と中心窩との距離が500μm以内であるときに視力転帰が有意に良好であった。栄養血管が検出できた症例には，これへの選択的光凝固を実施した。発症からの期間が短いときに視力転帰が有意に良好であった。

We reviewed 39 eyes of age-related macular degeneration successfully treated by laser photocoagulation. All the eyes showed choroidal neovascularization in the fovea before treatment. The eyes were followed up for 6 to 78 months, mean 13 months. The whole area of choroidal neovascularization was photocoagulated when the visual acuity was 0.2 or less. The final visual acuity was significantly favorable when the distance between the margin of neovascularization and the foveola was less than 500 μm. Selective photocoagulation of feeder vessels was performed when located outside the fovea. Posttreatment visual acuity was significantly favorable in recent-onset cases.
