走査型レーザー検眼鏡によるインドシアニングリーン螢光眼底造影の有用性と脈絡膜血管異常の検出

Detection of abnormal choroidal vessels by indocyanine green angiography using scanning laser ophthalmoscope

　眼底疾患患者および眼底に異常を認めない者92例115眼を対象に，走査型レーザー検眼鏡（以下SLO）にて，インドシアニングリーン螢光眼底造影（以下IA）を行った。SLOによるIAでは，脈絡膜血管の造影早期の経過が，動脈相，動静脈相，静脈相として明瞭に把握できた。このことから，動脈と静脈の鑑別が可能で，脈絡膜血管構築にはかなりの個人差のあることがわかった。また，blockによる低螢光と，脈絡膜毛細血管の充盈欠損あるいは遅延による低螢光との鑑別や，脈絡膜血管からの漏出の観察が可能であった。SLOによるIAは，従来のIAでは得られなかった詳細な所見が得られ，脈絡膜血管の異常を検出するうえできわめて有用であると考えた。

We performed indocyanine green angiographyusing a scanning laser ophthalmoscope in 115 eyeswith or without fundus disorder. We could differen-tiate the early angiographic phase of the choroidinto arterial, arteriovenous and venous phase. Itwas thus possible to identify an artery as differentfrom a vein in the choroid. In instances showinghypofluorescence, we could differentiate amongblockage, filling defect or filling delay in the chorio-capillaris. We also observed instances of dyeleakage from choroidal vessels. Indocyanine greenangiography using scanning laser ophthalmoscopeseemed to be of clinical value in the detection ofpathological choroidal vessels.
