網膜細動脈瘤による黄斑下血腫の外科的治療

Surgical removal of submacular hemorrhage secondary to retinal arteriolar macroaneurysm

　網膜細動脈瘤による厚い黄斑下血腫は予後不良とされている。そこで網膜細動脈瘤による黄斑下血腫6眼に対して組織プラスミノーゲンアクチベータおよびパーフルオロカーボンを用い外科的血腫除去術を行った。視力は6眼中4眼で6段階以上改善し（視力0.5以上は3眼），2眼で変化がなかった。変化がなかったもののうち1眼は術前より黄斑円孔を合併し，1眼は血腫が黄斑下で器質化していた。症例を選択すれば網膜細動脈瘤による厚い黄斑下血腫に対して本治療法は有効である可能性が示唆された。

We treated six eyes of submacular hematoma secondary to retinal arteriolar macroaneurysm by surgical removal of hematoma, tissue plasminogen activator and liquid perfluorocarbon. Surgery was performed within 16 days after onset. Visual acuity improved in 4 eyes including 3 eyes showing 20/40 or better. Visual acuity remained stable in 2 eyes, of which one eye had macular hole before surgery and the other had organized subretinal hemorrhage involving the fovea. Surgical removal of submacular hemorrhage may be beneficial if the patients are properly selected.
