Abstract 文献概要

　黄斑部に限局した梅毒性脈絡網膜炎を報告した。症例は51歳男性。右眼の視力低下と中心暗点を自覚し，黄斑部に約2乳頭径の灰黄色混濁が見られたため当科を紹介された。視力は，右0.3，左1.0。前房，硝子体に炎症所見はなかった。螢光眼底造影では初期から混濁網膜に不規則な過螢光が出現し，後期には組織染が示された。乳頭からの軽度の螢光漏出と静脈の壁染色が見られた。血液検査でTreponema pallidum hema—gglutination test 20, 480倍，Venereal diseaseresearch laboratory test 16倍であり，急性梅毒と考えられた。アンピシリン1日2gの投与を4週間行った。右眼黄斑部に軽度の色素上皮障害が残存したが，視力は1.2まで回復した。

A 51-year-old male presented with blurring and central scotoma in his right eye. Funduscopy showed grayish opacity of 2 disc diameters across in the macula. The visual acuity was 0.3. Fluor-escein angiography showed irregular hyperfluor-escence in the opaque area from the early phase and tissue staining towards the late phase. The retinal veins were slightly hyperpermeable to fluorescein. The left eye was unaffected throughout. He was diagnosed as acute syphilis because of seropositive findings. The visual acuity improved to 1.2 after 4 weeks of systemic ampicillin 2 grams daily.
