ぶどう膜炎様症状を呈した血管内リンパ腫症の1例

A case of intravascular lymphomatosis simulating uveitis

（展示87）　62歳男性に1か月前から頭痛，発熱，視力低下があり，内科を受診し，当科に紹介された。両眼の前房に軽度の細胞とフレアがあり，両眼眼底の後極部に漿液性網膜剥離があった。フルオレセインとインドシアニングリーンによる螢光造影で，脈絡膜の低螢光斑と，後極部の多発性螢光漏出があった。2か月後に全身状態が悪化して死亡した。剖検で全身の小血管内腔に異型リンパ球の増殖があり，血管内リンパ腫症と診断された。本症では，脈絡膜毛細血管内の腫瘍細胞の増殖が脈絡膜の循環障害を起こし、眼底病変が生じたと考えられた。

A 62-year-old male developed visual impairment, headache and fever since one month before. Both eyes showed cells and flare in the aqueous and serous retinal detachment in the posterior fundus. Fluorescein and indocyanine green angiography showed mottled hypofluorescent areas of the choroid, multiple dye leakage with dye pooling in the area of serous retinal detachment in the posterior fundus. He died of multiple organ failure 2 month later. Autopsy showed intravascular proliferation of atypical lymhocytes, leading to the diagnosis of intravascular lymphomatosis. It appeared that proliferation of tumor cells in the choroidal vessels induced disturbed choroidal circulation with fundus manifestations.
