涙腺腫瘤により発見されたシェーグレン症候群の1例

A case of Sjögren's syndrome detected by lacrimal gland enlargement

　症例は21歳女性。無痛性の右上眼瞼腫脹にて来院した。CTなどにより涙腺腫瘤を疑い，右涙腺腫瘤摘出術を施行した。摘出標本に著しいリンパ球浸潤と腺上皮の島状増生を認め，さらに小唾液腺生検，耳下腺造影，免疫学的検索をすすめ，同様の組織所見，耳下腺造影によるapple treelesion，抗SS-A抗体陽性の結果が得られ，シェーグレン症候群と確定診断できた。シェーグレン症候群の初発症状は眼の異物感や灼熱感などの眼乾燥症状および口渇症状などが多いが，本症例のような上眼瞼の腫瘤病変でも本症を念頭におく必要がある。

A 21-year-old female presented with painless swelling in the right upper eyelid. Computed tomo-graphy showed lacrimal gland tumor. Surgicallyremoved specimen showed lymphocytic infiltation and proliferation of lacrimal gland epithelial cells.

Biopsy of salivary and parotid glands showed similar findings. Anti-SS-A antibody was positive. These features led to the diagnosis of Sjögren's syndrome. The present case illustrates that lacri-mal gland tumor may be the initial clinical manifes-tation of this syndrome.
