高齢者に見られた多発性後極部網膜色素上皮症

Multifocal posterior pigment epitheliopathy in a senile person

　高齢者に見られた多発性後極部網膜色素上皮症を報告した。症例は80歳男性。右眼の黄斑部漿液性網膜剥離で発症し，下方の網膜剥離を生じた。裂孔は発見されず，眼内には炎症所見はなかった。網膜下液は体位変換によって移動した。螢光眼底造影で後極に色素上皮剥離と螢光漏出点が見られた。漏出点に対してレーザー光凝固を行ったが，経過中に新しい螢光漏出点が出現したため光凝固を追加し，網膜剥離は消失した。左眼後極には網脈絡膜萎縮巣が見られたが漏出はなかった。

A 80-year-old male presented with serous macular detachmant with nonrhegmatogenous retinal detachmant in the inferior hemisphere in the right eye. The subretinal fluid showed shifting. Fluorescein angiography showed numerous pigment epithelial detachment and dye leakages in the posterior fundus. Photocoagulation was performed to the sites of leakage. New dye leakages developed during the follow-up necessitating additional photocoagulations. Retinal detachment disappeared 6 months after the initial treatment. The left eye showed chorioretinal atrophic lesions without active dye leakage.
