ぶどう膜炎における網膜硝子体血管新生

Retinal and vitreal neovascularization in uveitis

　ベーチェット病651例中7例（1.1％）9眼，他のぶどう膜炎853例中3例（0.3％）3眼に網膜または硝子体中の血管新生がみられた。血管新生の部位は視神経乳頭5眼，後極部3眼，赤道部4眼で，それぞれ5眼，2眼，1眼で硝子体出血が生じた。乳頭と後極部に発生した新生血管は広範囲にわたる網膜の炎症性循環障害によるものと推測された。赤道部に生じたものは3眼が無血管帯，1眼は網膜前線維増殖によるものであった，ぶどう膜炎の発症から新生血管の出現までの期間は2か月から11年，平均2.8年，ベーチェット病だけ子は3.1年であった。乳頭および後極部に生じた新生血管は半数が治療により消失し，4眼ではまだ存続していた。血管新生眼の最終視力は，新生血管発生時と比較して，全体としてはあまり低下していなかった。

We observed retinal or intravitreal neovascular-ization in 7 out of 651 eyes (1.1%) with Behcet's disease and in 3 out of 853 eyes (0.3%) with other types of uveitis. Neovascularization occurred bilaterally in 2 cases of Behcet's disease. Newly formed vessels were located on the optic disc in 5 eyes, in the posterior fundus in 3 and in the equato-rial region in 4. Vitreous hemorrhage occurred in 5, 2 and 1 eye respectively in these groups.

Neovascularization from the optic disc and in theposterior fundus seemed to be secondary to circula-tory disturbances of inflammatory nature. Newly formed vessels in the equatorial region were as-sociated with preretinal fibrous proliferation in 1 eye and with avascular retina in 3 eyes. The inter-val between onset of uveitis and neovascularization ranged from 2 months to 11 years, average 2.8 years. Newly formed vessels from the optic disc and in the posterior fundus disappeared after antiin-flammatory treatments in 50% of the cases. The presence of vitreoretinal neovascularization did not seriously affect the visual acuity.
