桐沢型ぶどう膜炎31例の臨床的観察

Clinical features of 31 cases with Kirisawa-type uveitis

　大阪大学眼科で1973年から1989年までに経験した桐沢型ぶどう膜炎31例35眼について統計学的考察を行った。男性20例22眼，女性11例13眼で，平均年齢は45.1歳であった。網膜壊死の範囲が広い症例は網膜剥離を起こすものが多く，視力予後も不良であった。1973年から1981年の前半と，1982年から1989年の後半とに分けてみると，網膜剥離の発生は前半で12眼中9眼，後半では23眼中13眼で，最終視力が0.1以上の症例は前半では9眼中0眼，後半では13眼中4眼であった。ステロイド治療にアシクロビルの併用療法を行った症例は，最終視力0.1以上を維持している症例が多かった。ステロイド投与量についてみると，投与総量がブレドニゾロン換算1,500mg以上の症例では7眼が視力予後良好であるのに対し，1,500mg未満の症例では1例であった。水痘帯状ヘルペスウイルスに対する抗体率の高かった3例は，すべて中高年男性で片眼性であり，視力予後は不良であった。単純ヘルペスウイルスに対する抗体率の高かった5例では若い女性が3例を占め，視力予後の良好なものもみられた。

We observed a total of 31 cases with Kirisawa type uveitis, or acute retinal necrosis, from 1973 through 1989. The series comprised 20 males, 22 eyes, and 11 females, 13 eyes. The ages averaged 45. 1 years. Cases with extensive retinal necrosis were prone to develop retinal detachment with poor visual outcome. There were 12 affected eyes before 1981 and 23 eves after 1982. Retinal detachmentdeveloped in 9 eyes in the former group and in 13 eyes in the latter. None of the 9 eyes and 4 of the 13 eyes retained the visual acuity of 6/60 or better. Treatment with systemic corticosteroid and acy-clovir contributed to the final visual acuity of 6/60 or more. Fair visual acuity resulted in 7 eyes treat-ed with more than 1,500 mg of equivalent of pred-nisolone and in only one eye treated with a lesser simplex virus ; HSV, or varicella zoster virus;VZV, in the vitreous and the serum in 9 cases. Five cases showed high titers to HSV and 3 cases to VZV.
