要約 目的：虹彩毛様体囊腫に伴う狭隅角眼に対して白内障手術を施行し，術後隅角開大がみられた症例の報告。
症例：78歳男性。両眼の視力障害を主訴に受診した。初診時，左眼に虹彩囊腫，両眼に核白内障がみられた。
所見と経過：矯正視力は右0.4，左0.5，両眼にEmery-Little分類Grade 3の核白内障を認めた。術前眼圧は右20mmHg，左21mmHg，UBMにて左眼下耳側に虹彩毛様体囊腫に伴う閉塞隅角を認めた。白内障の手術後に隅角は開大し，経過観察期間中に明らかな眼圧上昇はなかった。
結論：虹彩毛様体囊腫に伴う狭隅角眼に対して，白内障手術後に隅角が開大した。以後，隅角閉塞はなかった。
Abstract Purpose: To report a case who showed iridociliary kuncyst and who showed widening of anterior chamber angle following cataract surgery.
Case: A 78-year-old male presented with impaired vision in both eyes as the chief complaint.
Findings and Clinical Course: Nuclear cataract was present in both eyes. The left eye showed iris cyst with narrow chamber angle. Corrected visual acuity was 0.4 right and 0.5 left. Intraocular pressure was 20 mmHg right and 21 mmHg left. Ultrasound biomicroscopy（UBM）showed closed chamber angle in the inferior temporal quadrant presumably due to cyst of ciliary body. The chamber angle widened following cataract surgery with no elevation of intraocular pressure during one year of follow-up.
Conclusion: Cataract surgery in an eye with iridociliary cyst and narrow chamber angle was followed by persistent widening of anterior chamber angle.
