内外同時眼窩切開により摘出した眼窩内静脈瘤の1症例

A case of excised orbital varix by combined medial and lateral orbitotomy

　眼窩内静脈瘤が筋円錐内に存在し，内外同時眼窩切開にて摘出し，良好な視力予後を得た1症例を経験した。症例は48歳女性で右眼球突出，視力低下を主訴とし紹介受診。CT, MRIより筋円錐内に腫瘤を認めたが，臨床症状，検査所見より悪性像は得られなかった。入院管理後症状が悪化し，内外同時眼窩切開による腫瘍摘出術を施行した。眼窩内静脈瘤が筋円錐内に存在する場合，手術による治療は合併症の発生率が高く，手術予後は必ずしもよくないといわれ保存的治療が主体とされているが，今回施行した方法は，術野を安全にかつ十分得ることが可能であり，今後症例によっては積極的に行ってもよい手術術式であると考えられた。

A 48-year-old female presented with proptosis and acute visual loss in the right eye of one month' duration. A tumor mass was detected within the muscle cone by computed tomography and magnetic resonance imaging. There was no sign of malignancy. The orbital tumor was removed by combined medial and lateral orbitotomy. The tumor was identified as orbital varix. The surgery resulted in good visual outcome.
