腫瘍摘出術を行った拡大傾向の著明な虹彩黒色細胞腫の1例

A case of rapidly enlarging melanocytoma of the iris treated by excision

　60歳女性が右眼の虹彩腫瘍で紹介され受診した。腫瘍は色素が豊富で，直径は2.5mmであり，角膜後面に接触していた。以後14か月後に腫瘍は1.5倍に増大した。悪性黒色腫の可能性と角膜内皮細胞などへの悪影響などを考慮して腫瘍を摘出した。病理診断は虹彩黒色細胞腫であった。拡大傾向のある虹彩黒色細胞腫は悪性黒色腫との鑑別が困難であり，角膜内皮や水晶体に合併症が生じる可能性がある場合には摘出手術が望ましいことを示す症例である。

A 60-year-old female presented with an iris tumor in her right eye. The tumor was richly pigmented, had a diameter of 2.5 mm and was in apposition to the posterior corneal surface. It enlarged by 50% 14 months later. We excised the tumor because of the possibility of malignant melanoma and of damage to the corneal endothelium and the lens. Histopathological studies showed the tumor to be melanocytoma of the iris. This case illustrates that timely excision is indicated when malignant melanoma cannot be ruled out and when damage to the corneal endothelium or the lens is imminent.
