エキシマレーザーによる近視矯正手術後の細菌性角膜炎

A case of microbial keratitis after photorefractive keratectomy by excimer laser

　32歳男性がエキシマレーザーによる近視矯正手術を一眼に受け，術直後から治療用ソフトコンタクトレンズを装着していた。術後5日目に手術眼に違和感が生じて当科を受診した。矯正視力は1.2であり，角膜の鼻側上部に，角膜実質に炎症性細胞浸潤を伴う角膜炎がみられた。細菌性角膜炎としてただちに抗生物質の点眼と内服を開始した。2週間後に角膜炎は改善し，軽度の角膜白斑が残ったが，視力は良好であった。初診時に行った結膜分泌物とコンタクトレンズの培養検査で，後者からcoagulase—negativeのブドウ球菌が検出された。

A 32-year-old male had undergone photorefractive keratectomy by excimer laser in one eye. He had been wearing therapeutic soft contact lens immediately after the treatment. He visited us 5 days after surgery with foreign body sensation in the treated eye. The affected eye showed inflammatory infiltration in the corneal stroma. The visual acuity was 1. 2. Treatment with topical and systemic antibodies resulted in cure two weeks later. Culture of eye discharge and contact lens showed coagulase-negative staphylo-coccus in the latter.
